To schedule a new patient appointment:
To schedule a new patient appointment, please call 816-587-4900, and choose ext 3 for scheduling. Please leave a message for scheduling to contact you back. The outgoing message asks for your name, or the name of the patient, the patient’s date of birth, the name of the EAP or Insurance, the reason to be seen, and your best contact #. We are unable to return calls without all the requested information. We are not able to schedule an appointment with anyone other than the patient if they are over the age of 18 – unless you have a Power of Attorney. If you are calling to schedule a minor, you must be a bio-parent or legal guardian; documentation will be required at the time of the session. Please allow two business days for a return call.

To cancel or reschedule an existing patient appointment:
Scheduling is only able to schedule the initial intake appointment. To cancel or reschedule an existing appointment you will need to contact your provider directly. Please call 816-753-3333 and listen for your provider’s extension.

Initial intake paperwork:
If you haven’t received your new patient intake forms within 48 hours of scheduling, please check your junk/spam folder for an email from @mapskc.net. If you still do not see the email, contact your provider to have them resent. If you require assistance with the forms or have questions about the forms, please contact your provider directly at 816-753-3333 and listen for your provider’s extension. Scheduling is not able to help with intake paperwork.
