In order to get started, you'll first need to fill out the intake forms found at the link below.

If your appointment is face-to-face, this paperwork can be printed, filled out, and brought to your first appointment. You can also fill out the paperwork online, emailed to you within 48 hours of scheduling. Please allow the 48 hours and check your junk and spam folder before asking to have it resent. Otherwise, you can contact your provider directly to have it sent.
